Annual Report

Annual Reports are due from all COSMA Program Members, by July 31, every year.

​ANNUAL REPORT DOCUMENT 2023-24

PROGRAM MEMBERSHIP DUES INCREASED BY TEN PERCENT FOR THE FISCAL YEAR 2024-25. HOWEVER, ALL PROGRAMS' FINANCIAL POSITIONS WILL BE ACCOMMODATED. SEE SECTION 3 OF THE ANNUAL REPORT.

Program members

• Complete Section 1 only.

Programs in Candidacy Status and Accredited Programs

• Complete Sections 1 and 2.

Optional: Section 3: Budget Chart and Flowchart (Optional for Programs requesting fee reduction)

Failure to submit a completed Annual Report or Membership fee results in automatic Administrative Probation. A $198 fine will be assessed for late payment or Annual Report.
