Annual Reports are due from all COSMA Program Members, by July 31, every year.

• Complete Section 1 only.

Programs in Candidacy Status and Accredited Programs

• Complete Sections 1 and 2.

Optional: Section 3: Budget Chart and Flowchart (Optional for Programs requesting fee reduction)
Need an extension? Complete page 12 of the Annual Report.

Failure to submit a completed Annual Report or Membership fee results in automatic Administrative Probation. A $200 fine will be assessed for late payment or Annual Report.
