How to Join
Electronic Applications:
To join COSMA, complete the appropriate application form. Three application forms are available.
- Application for COSMA Institutional Membership & Candidate for Accreditation Status (Institutions with US-based Regional Accreditation)
- Application for COSMA Institutional Membership & Candidate for Accreditation Status (Institutions without US-based Regional Accreditation)
- Application for individual membership

To join COSMA, complete the appropriate application form. Three application forms are available for download if you wish to print and mail.

Payment can be made by credit card (Visa or Master Card), or a check can be mailed in along with the application form. Note that your membership needs to be renewed each year.
COSMA memberships are based on a July 1 through June 30 fiscal year.
